Q:  I got so tired of fighting with my inattentive/ hyperactive child about chronic laziness different values, and not really trying that medication seemed the best option to restore some level of peace at home.  Medication has not lived up to my expectations.  Realistically, what can I expect medication to do and not to do?

A:  Many children as well as their families want to believe that a "quick fix." is available.  However, the key remains in the attitude of the individual.  Just as an operation improves the chances of walking, it's consistent effort in many hours of physical therapy that makes it happen.

MEDICATION CAN HELP IN THE AREAS OF FOCUS AND DISTRACTABILITY:

  • Improvement in Academic Performance is often seen in quiz/ test scores, visual memory and handwriting.  There can be an increase in the accuracy of work amount and of work completed.  There can be an improvement in the attention available for special instruction.  Medication can also decrease frustrations with school work in general.
  • Behavior can improve by increased compliance with rules & responsiveness to punishment; increased delay of gratification; effectiveness of rewards; and the ability to sit still longer. Medication can also decrease the overall activity level.
  • Organization can improve by an increase in more effective of time.
  • Social Skills can benefit from decreased aggression, impulsivity, disruptiveness, negativity, & reactivity.  Medication can also increase the ability to "fit in" socially.

MEDICATION DOES NOT HELP CHANGE ATTITUDES OR MOTIVATION:

  • Academic Performance will not reflect improvement in achievement test scores; concentration; replace core skills missed in the past; change defeatist attitudes, or correct learning disabilities.
  • Behavior will not include the willingness to conform or comply; or motivate to attain goals.  Decision-making skills; where to place attention; or how to relax are often taught in counseling.
  • Organization will not be automatic or include the ability to transition gracefully.
  • Social Skills will not improve conversation or ease in getting along with others.  The areas of self-esteem and confidence are outside the scope of medication. 
  • Inattentive/ hyperactive medication does not usually improve sleep; or reduce the stigma of taking medication.

There is no "magic pill" for inattentive/ hyperactivity or mental health issues in general.  You can see by the list above that medication is only half of the answer.  If medication is appropriate, the on-going combination of monitoring by a Psychiatrist for tolerance and effectiveness, counseling, personal determination, as well as support from family, friends, school and community form the foundation for long-term success.
